Once you complete your online booking, I will email you a link to the Cat Behavior Questionnaire. The questionnaire helps me get to know your cat's history, environment, and behavior so we can spend our time together focusing on your specific concerns.

If you've booked the Comprehensive Consultation Package, please complete the questionnaire in full at least 72 hours before our scheduled consultation. This allows me time to review your responses and, if needed, send additional questionnaires to gather more detailed information about specific behaviors.

If you've booked the Wellness & Prevention Package, simply complete the sections that are relevant to your cat and your goals at least 48 hours before our appointment. Additional questionnaires are usually not necessary for this package.
